We offer a simple subscription plan that includes all features, all support and
all upgrades for an entire year. During the year you are subscribed, you are allowed to generate an unlimited number
of reports for an unlimited number of your patients.
The Terms of Service prohibit multiple doctors from using the same account.
Do you have special needs that are not met by Terms of Service? Please contact

To purchase a subscription, please use the Log In link above to create an account (if you have not already done so).
Then, you will be prompted for your billing information. Once you have made the purchase, every time you log in,
the report generator will be shown.
When your subscription expires, the next time you log in, you will be prompted for your billing information again.
